Proof of vaccination, a recent negative Covid rapid test within the past 72 hours or a medical exemption is required as per public health orders.

1. Player’s name
2. Contact email
3. An indiction of which sessions you will be attending (you welcome to come to all)
4. Player’s proof of vaccination, medical exemption or indiction that you will email a recent negative Covid rapid test to serenasvidal@gmail.com at least 24 hours before clinic. 

Also, please bring your proof of vaccination, a recent negative Covid rapid test within the past 72 hours or a medical exemption to the clinics.
